Recently David Jaffe, the producer of Twisted Metal, posted some videos on his blog revealing a few alternate endings for Sweet Tooth, the franchise’s flagship character (via Eurogamer).
Originally the latest PS3 Twisted Metal reboot was slated for a Teen rating which forced the development team to tone down the gore the series is so well known for. In the end the game ended up with a mature rating which allowed for Jaffe to go with more brutal endings.

The first alternate ending shows Sweet Tooth get his head decapitated by Sophia after he falls deeply in love with her.
The second alternate ending shifts some of the existing plot around so we see the hospital massacre scene take place later in the game. Sweet Tooth decides to throw his equally as deranged daughter out of a window after she convinces him that there may still be some good in him.
Jaffe also talks about the future of the Twisted Metal franchise later in the post when he said that this would be the last Twisted Metal game for some time. “Hope the TM fans out there have dug some of the ‘what could have been’ stuff from what I am fairly certain will be the last Twisted Metal game made. At least for a very, very long time!”
